AnswerWith the loss of MP stability, other forces in the MP produce the characteristic ulnar drift. For example, the collapse of the wrist contributes to the ulnar drift. Debilitated radiocarpal ligaments cause radial rotation of metacarpals and carpus on the radio, resulting in the ulnar deviation of the MP joint through the Z mechanism. This phenomenon describes the consequence of an imbalance of opposing forces in a joint; when a particular articulation is continually waning in one direction, the joints on each side will adopt the opposite position if there are no physical impediments present. [] In addition, extended tendons can be moved or delocated in the ulnar direction and contribute to the ulnar drift. The normal shift in ulnar of the fourth and fifth tendons of the extender during the bending of the MP joints is exaggerated when the radial side of the transverse lamina fibers is stretched as a result of synovial proliferation. This was done by reviewing the literature of articles published on the mechanical etiology of the ulnar drift. There are several theories about the cause of the ulnar drift, however, still lacks conclusive evidence. Current mechanical factors that are postulated to play a role include: collateral ligaments failure, intraarticular pressure changes, degenerative changes in carpian anatomy and metacarpia, induced muscle hypoxia changes in wrist tension and exacerbating activities of daily life. Although the current theories on ulnar drift almost universally include at least partially mechanical logic, the causes can be multifactorial. Significant research is needed to clarify the relative importance of mechanical factors leading to concurrent significant ulnar drift with advanced rheumatoid arthritis.
